To apply for public housing you can stop by their office located at: 206 E. Washington St., Toledo, IL 62468 to pick one up, or you can call the office at 217-849-2071, to have an application mailed to you, during normal office hours.


Once you have completed the application you must hand deliver it to the above address during normal office hours.  Be sure to bring copies or originals of birth certificates, social security cards, proof of income, and photo IDs for all adult household members.
