• Stop in office to pick up application; address: 433 Baltimore Ave., Clarksburg, WV between 9 am - 3 pm on Tuesdays and Thursdays only.
• Download, print, and complete

Bring these documents: 

• social security cards
• proof of all income
• birth certificates
• photo IDs for all adult household members

Applicant must hand deliver on Tuesdays and Thursdays between 9 am-3pm.
